Colon and rectal carcinoma is important focus of the division of colorectal surgery. We offer highly specialized workup for colorectal cancer to accurately stage the extent of the tumor. Rectal ultrasonography and MRI imaging allow precise preoperative evaluation of tumor size and depth of penetration of the tumor and an assessment of lymph node involvement. A newer technology recently acquired by USC, a rectal MRI coil, may allow even better imaging of the tumor and nodes and is routinely employed in the evaluation of rectal cancer. The availability of this technology allows for optimal workup of patients with colorectal cancer so that appropriate treatment strategies individualized for the patient can be developed, giving them the best chance for cure.
Multiple specialized surgical treatment options are available for patients with colon and rectal cancer at USC. These include transanal excisions, transanal endoscopic microsurgery, low anterior resections, abdominoperineal resections, and colonic J-pouch reconstruction. Many of these procedures can be performed laparoscopically or with minimally invasive techniques. Our overall laparoscopic experience remains one of the most extensive in the world. Our team led by Dr. Anthony Senagore has amongst the world’s largest experience in all forms of laparoscopy (including new 3-dimensional viewing), ultralow (sphincter muscle-preserving) rectal cancer surgery, and recurrent colorectal cancer.
We also offer multidisciplinary collaboration with medical and radiation oncologists, urologists (for advanced tumors involving the bladder or prostate), gynecologists (for tumors at risk of involving the uterus or ovaries), and hepatobiliary surgeons (for liver metastases). We are one of few centers that offer combined colorectal and liver surgery, to minimize the need for multiple surgeries – which has been shown to decrease overall complications. In conjunction with our radiation therapy colleagues, the team also provides intraoperative radiation therapy when indicated for complex tumors. Our goal is to surround the patient with a world-class team of experts and tailor each person’s care to maximize the chance of cure.
