Appointments
For Patients: How To Make An Appointment
To make an appointment with a specialist at the Center for Colorectal and Pelvic Floor Disorders at USC, you may call our office at (323) 442-6860 during the hours of 8:00 am - 5:00 pm PST, Monday through Friday. Our clinical assistants will answer your questions, collect the necessary information required for pre-registering, and schedule an appointment. Please understand that we are not able to offer medical advice over the telephone or through the Internet or mail.
Information Needed:
- Your personal contact information (phone number, alternate phone number, address, date of birth, and social security number)
- Diagnosis
- Name and phone number of the referring physician and your primary care physician
- Your insurance information, including any necessary HMO authorization
Our specialists request that you bring certain medical information to your first visit. This includes x-rays, cat-scan films/reports, and pathology slides/reports, surgical reports and a summary of your treatment to date.
We will send you an orientation packet prior to your appointment.
If you are being seen by one of our specialists due to loss of bowel control (fecal incontinence), questionnaires need to be completed. You may download the following, complete, and bring with you to your appointment.
What To Bring to Your Appointment
Please bring the following items to your appointment:
- The information requested in the orientation packet.
- Health insurance information including your insurance card(s), special forms, referrals or important phone numbers.
- If you are traveling from out-of-town, please provide contact information of where you will be staying in the Los Angeles area.
- Any additional medical records not already sent in advance of your visit.
- A list of all your physician’s names, addresses and their telephone numbers.
- A list of any medications that you are currently taking, along with the prescribed dosages.
