Physicians may refer patients to the Center for Colorectal and Pelvic Floor Disorders by calling our office at: (323) 442-6860.

Patient Information Needed at the Time of Referral

  • Patient name and contact information
  • Diagnosis
  • Pertinent medical records
  • Patient's telephone number
  • Patient's insurance information, including any necessary HMO authorization

Patients should bring supporting documentation (x-rays, cat scan films/reports, pathology slides/reports, etc.) to their first appointment.

After a Referral is Made
Our office will call your patient for pre-registration and discuss what he or she can expect at the first visit. An orientation packet will be sent to your patient prior to the appointment with pertinent information about the visit, and logistical information, such as location, directions, and parking.

A specialist will contact referring physicians to discuss the results of the consultation visit. We encourage your participation in every phase of your patient's care.
