Illness usually follows Pareto's principle: 80 percent of the trouble comes from 20 percent of the problems. When applied to the practice of medicine, it appears that 80 percent of illnesses are readily recognizable, easily treatable, have a predictable outcome, can be managed by most physicians, and are unlikely to present problematic recovery. On the other hand, 20 percent of the illnesses are difficult to recognize: their treatment is debatable, their outcome is unpredictable, their management is strongly physician dependent and problems during recovery are the rule rather than the exception. We have formed the Thoracic-Foregut Group to identify these "20 percent" patients with complicated foregut and pulmonary problems, and utilize our collective expertise in their treatment.
